File a Grievance

Grievance Reporting: Upholding Standards of Ethical Care

At the Minnesota Association of Sober Homes (MASH), we are committed to protecting the dignity, safety, and wellbeing of all residents in certified sober homes. If you believe a MASH-certified recovery residence has violated ethical or operational standards, you may submit a formal grievance through the form below.

Please note: Before filing with MASH, we strongly encourage individuals to first attempt to resolve grievances directly with the residence operator. If that is not possible or appropriate, MASH will review all submitted complaints in accordance with our internal review process.

How Grievances Are Handled

  1. Submission: Grievance submitted through secure online form.

  2. Review: MASH Ethics Committee will review within 7 business days.

  3. Follow-Up: You may be contacted for additional information.

  4. Outcome: Appropriate actions may include mediation, operator education, disciplinary actions, revocation of certification, and/or escalation to licensing authorities, if necessary.

  5. Documentation: All grievances and resolutions are documented to ensure accountability.
